A "Welfare Plan" proposal By-Laws was adopted by this Local Union on April 27, 2000.
The welfare committee would like to take a poll as to how many members will be interested
in joining this fund.  To be eligible for this fund you must be a member in good standing, pay
the initiation fee of $25.00 and have made 6 months contribution to the fund.  The contribution
fee will be $10.00, which will be reviewed by the committee within 6 months of the fund being
opened for determination on a decrease or increase to the donations.  Please inform the union
hall by phone or e-mail as to your interest by the end of September.

**NOTICE**   Any person on Long Term Disability (LTD) should NOT receive a check from
the company they work for but from an insurance company, which provides your employer's
LTD coverage.  If you receive a check from your employer, you will have to refund it.

Each employer has a policy determining how LTD works.  Most policies are during the 1st 24
months of LTD a person is carried on the company's report as still being active, accruing seniority
& retirement credit & any insurance plans you have through your employer will remain as is.
After 24 months the employer then carries you on their records as terminated, this is only for
record keeping, you will remain on LTD as long as eligible (a person can remain on LTD up to age
65, if eligible). 

If your doctor & the employer's doctor decide you are able to return to work during the employers 
active period, with no restrictions, then you will be returned to the status you left at if a job is available, 
however, if you become able to work after the active period ends, then you will be considered a new
applicant for a job, with the possibility of not being hired.

If your doctor has informed you he is releasing you to return to work during the employer's active
period contact the union hall first, NOT THE LTD INSURANCE CARRIER, because if the
employer's doctor does not agree with your doctor, you have jeopardized your LTD status by 
notifying the LTD company you are no longer disabled.  Remember the LTD carrier & the employer
are totally different & the LTD carrier is NOT who decides you can return to your present job, with
your present employer.

If you are on LTD DO NOT work another job unless it has been approved by the LTD company
as REHABILITATIVE are you run the risk of being terminated for LTD coverage.
